How can I access these services?
A physician referral is required for evaluation and treatment from an occupational therapist, speech therapist, or initiation of cardio/pulmonary rehabilitation and the Medical Exercise Program.
A physical therapist may evaluate and treat either by physician referral or self-referral. There are certain restrictions placed on this by various insurance carriers. Please contact Outpatient Rehabilitation for the specific guidelines for self-referral.
What can I expect from an evaluation?
You will meet one-on-one with the therapist for your evaluation. From this evaluation, you and your therapist will decide the appropriate course of treatment that will allow you to meet the goals that you wish to attain.
