Dear Providers:

This document discusses more detail about Test Results, how they will release, and how comments can be made. This document will impact Ambulatory (SVPN) physicians the most. Please contact x3900 or email SV Clinical Informatics (
UPDATE for all care providers who will see patients:
A parent or someone who is a legal representative caring for a child or adult can be granted access to see the dependents information. This is referred to as Proxy Access. It can only be obtained by the parent or representative physically bringing the required documentation to our HIM department. Please direct anyone inquiring about proxy access to HIM. 
UPDATE regarding documents patients can print from MyChart and may present to you:
The Continuity of Care Document references information specific to an inpatient encounter.  
There is another document titled Patient Health Summary which is specific to an outpatient clinic visit.  An example of uses for the Patient Health Summary (outpatient version) – patient can update their medication list, print this form, and bring it to the office visit.  
*NOTE:  Access to the documents is located in different places on MyChart, however; please realize the difference in title does mean the form includes different information. The name of the document appears in top left hand corner and documents look very similar at a glance.
