Maximizing our health system’s performance relative to value-based purchasing. Medicare introduced this new payment system last year shifting its focus from payments based on volume to payments based on performance. Scoring of core measures, patient experience, hospital acquired conditions and readmissions determines whether hospitals are penalized, break even or rewarded.
Accelerating the implementation of best practices to ensure excellent patient experiences systemwide. As part of this initiative, we have implemented an after-discharge call system to follow up with patients and other efforts that focus on the basics of creating a safe environment and improving communications.
