High-Risk Follow-up Clinic
The High-Risk Follow-Up Clinic program serves children who have graduated from the SVH NICU, helping with any needs they may have following hospitalization. The clinic team consists of a pediatrician, pediatric physical therapist, pediatric occupational therapist, pediatric nurse and a pediatric care manager.
The parents of all babies discharged from the NICU who meet the criteria for follow-up are contacted about participation in the clinic. Children may also be referred by other sources such as directly from their primary care provider, from local service agencies (STEP, ECI) and also directly from the family (with approval of their primary care provider). A Family Resource Coordinator (FRC) from either STEP or ECI attends each clinic.
