At St. Vincent Healthcare, we respect the right of the individual to make choices according to your own wishes. Five Wishes ® provides an opportunity to document your personal requests for your own health care choices now and through end of life. The Five Wishes ® Advance Directive booklet is the tool of choice used at St. Vincent Healthcare to help patients with the completion of Advance Directives.
What is an Advance Directive?
An Advance Directive is a plan indicating preference for future healthcare decisions if you are unable to make decisions. It is generally a written, legal document such as a Living Will, Durable Power of Attorney for Healthcare, or Comfort One (a do not resuscitate order specific to the State of Montana).
The Five Wishes You Can Make
- The person I want to make care decisions for me when I can't.
- The kind of medical treatment I want or don't want.
- How comfortable I want to be.
- How I want people to treat me.
- What I want my loved ones to know.
